Project Details

SCOPE (Seamless Care Optimizing the Patient Experience) is a shared virtual interprofessional care team for primary care providers (PCPs) in greater Toronto regions who would benefit from access to a team approach for patient care (preference will be given to unaffiliated physicians). SCOPE is a platform that empowers collaborative work between primary care providers, hospital services and community health partners while fostering a primary care community of practice. We created a website utilizing their current logo for the overall look and feel for both patients and providers.
